*Region Benchmark: RRH compares our results including CARE Tool and Discharges to home to other rehabilitation hospitals in the region. Discharge to home includes returning to any of the following: home with or without assistance, assisted living facility, boarding home, or transitional living facility.

*CARE Tool: The Continuity Assessment Record and Evaluation (CARE) Item Set: As part of the Medicare Post-Acute Care Payment Reform Demonstration (PAC-PRD), a standardized patient assessment tool was developed for use in acute hospital discharge and at post-acute care admission and discharge. Higher the score the better.
