Upon admission the patient will be assigned
a physician to follow them through the rehabilitation
process. The patient's referring doctor
will then be consulted to follow the patient's
care on a as needed basis. A social worker will work with the patient during their stay to review progress in rehab, to assist with discharge planning and community resources, and to serve as a source of support to the patient and family.
If the patient is unable to tolerate the rehab program or has reached a plateau in their progress, it may be recommended that a referral be made to a less intense rehabilitation environment. This transfer will allow the patient to continue their recovery at a pace that meets their specific needs. Recovery does not end at discharge. Depending on the patient's particular functional needs, therapy may continue through home health or outpatient therapy services.
What to Do
It is helpful if a family member can be
present during the admission process. Family
members are asked to bring the following
items on the day of admission:
- Loose-fitting clothing such as pants,
shirts and sweat suits
- Non-skid shoes
- Personal toiletries including deodorant,
toothpaste/brush, make-up, electric razor,
etc.
- Assistive devices such as dentures,
hearing aids & glasses
- List of medications taken at home (only
applies to patients admitted from home)
- Medical insurance information (only
applies to patients admitted from home)
