What You Can Expect from Us as a Client
To be treated with respect and consideration
- Our program does not discriminate based on sexual preference, gender, race, age or religion.
Confidentiality
- Although the program requires the sharing of certain information with outside entities for treatment, payment and healthcare operations, your information will be treated with privacy in accordance with HIPAA guidelines.
What We Expect of You
- Treat program staff with respect and consideration.
- Provide accurate and current income documentation upon request and yearly enrollment.
- Give program staff adequate time to work on requests.
- Be completely honest.
- Attend appointments and follow doctor’s orders.
If you are able, payment towards your care is greatly appreciated.
